Then, the system prioritizes needs and provides a Personalized Report that guides the caregiver on what they need to know and where to go for help.<\/p>\n<p>The report links directly to resources on the MAP website, which is integrated with the 211 Community Resource Database. The Checklist was developed through a partnership with the Johns Hopkins University School of Nursing, the Maryland Department of Aging, and the Maryland Information Network, which powers 211.<\/p>\n\t\t\t<a href=\"https:\/\/marylandaccesspoint.211md.org\/memorycare\/?utm_source=Aging&#038;utm_medium=Referral&#038;utm_campaign=211Resources&#038;utm_content=\"  target=\"_blank\" rel=\"noopener\">\n\t\t\t\t\t\t\tUse the Checklist\n\t\t\t\t\t<\/a>\n\t\t<h3>Screening Tool<\/h3>\n\t\t<p>If the patient doesn&#8217;t have a diagnosis, the <a href=\"https:\/\/marylandaccesspoint.211md.org\/ad8\/\" target=\"_blank\" rel=\"noopener\">AD:8 Cognitive Screening Tool<\/a> compares signs of normal aging to potential signs of mild cognitive impairment.
